2011 Champions in Health  2011 Champions in Health Awards Celebration - February 17, 2011
On Thursday, February 17, 2011, Community Health Charities will host their 4th annual Champions in Health Awards Dinner and Celebration. The event will be held at the Embassy Suites at Cool Springs in Franklin, Tennessee where "Volunteers of the Year" representing member health charities will be honored. Additionally two "Leader in Health" awards will be presented to an individual and a corporation nominated by the community for outstanding contributions to the health industry in 2010. Below you will find the 2011 nomination forms.
